Copy of Education certificate (we can accept six grade diploma as a minimum)

Copy of Pregnancy Test (applies to female applicants only)
Two recent color 3cmx4cm photographs (white background and white shirt)
Parents Legal Authorization (only applies for minors under 21 years old)
Note 1: Before enrolling for the Basic Maritime Safety Course, the seafarer candidate must perform a Swimming Test which includes a blood pressure test
Note 2: The enrollment application will be given to the seafarer candidate that has approved the Swimming Test so that it can be filled out with the required information,
Note 3: In case of women, they must present a current pregnancy test before starting this activity
